Community Health Assessment Underway in the Township of Bloomfield

Bloomfield, NJ – The Township of Bloomfield Department of Health (BDH), in conjunction with  the Bloomfield Board of Health, is initiating its next Community Health Assessment (CHA) in partnership with its community partners. Assessment of a community’s health is one of the core functions of local health departments whereas the last CHA for the BDH was completed in 2017-2018. Over the next several months, BDH, is looking for community members to participate in answering the new health assessment. This assessment is in conjunction with Strategic Health Advisers, LLC & the Bloomfield Community Health Impact Coalition, who will be working towards compiling an updated CHA document.

Community-based organizations participating in the Bloomfield Community Health Impact Coalition include Bloomfield Public Library, Partnership for Maternal and Child Health of Northern New Jersey, Mental Health Association of Essex & Morris, Mountainside Medical Center, Clara Maass Medical Center, Essex-Passaic Wellness, Bethany Presbyterian Church, Bloomfield School District, Adapt/Family Connections, Bloomfield College, Zufall Health/FQHC, etc. Township of Bloomfield agencies participating include Departments of Health, Human Services and Recreation.

The aim is to identify key health assets, needs and disparities in the Township through comprehensive data collection, research and analysis. BDH aims to obtain a complete understanding of public health issues affecting Bloomfield residents/communities and collect accurate data that can be used to develop strategies to improve the health of its residents. BDH encourages Township residents to participate in this process by taking the “Community Voices Survey”, which will be available in English, Spanish & Haitian Creole. This survey will take approximately 15 minutes to complete and all responses will be kept confidential. In addition to these surveys, BDH is working with community leaders to ensure all voices are heard through focus group and key informant interviews. An assessment of community partnerships will also be included in the health assessment process.

The BDH encourages everyone to participate in this survey. The more participation the more effective the data will be in planning future health initiatives in the Township. This survey can be accessed at the BDH webpage here and use the QR Code below.
