CSPNJ Launches Housing Supports Program for South Jersey Residents

Medicaid Initiative Funded by a Grant from The New Jersey Department of Community Affairs & Department of Human Services

FREEHOLD – May 28, 2026 – Collaborative Support Programs of New Jersey is now actively accepting referrals as part of NJ’s housing services community as a Housing Supports Program provider in Atlantic, Camden, Cape May, Cumberland, Gloucester and Salem counties, in support of the Camden Coalition. Once fully operational, CSPNJ will have the capacity to support over 100 individuals concurrently with housing stability services across all five Managed Care Organizations: Aetna®, Fidelis Care, Horizon Blue Cross Blue Shield, UnitedHealthcare, and Wellpoint.

The Housing Supports Program helps individuals facing housing insecurity who have Medicaid find and maintain housing. First initiated in June of 2025, this program offers four key services: pre-tenancy, tenancy sustainment, move-in supports, and residential modification and remediation. CSPNJ has six staff members trained and available to provide Housing Support services. In its current stage, a caseload of around twenty people served is anticipated.

“We’re incredibly grateful to be joining this network of providers to open new opportunities for safe and secure housing for the folks in our communities,” CSPNJ Deputy Director of Southern Region Niki Schlett says. “With the housing epidemic continue to rise, it is crucial for organizations like CSPNJ and others to be able to show up for those needing assistance in avoiding or recovering from homelessness with a compassionate approach.”

CSPNJ will offer focused, time-limited support services to individuals in the community designed to help them prepare for, obtain, and maintain stable housing. Through a person-centered approach, CSPNJ will work alongside individuals to address barriers to housing stability, strengthen independent living skills, connect participants to community-based resources, and promote long-term wellness and self-sufficiency. Services may include housing readiness support, assistance with navigating housing applications and systems, coordination with landlords and service providers, and ongoing guidance aimed at helping individuals successfully transition into and sustain safe, stable housing within the community.

Among the five community wellness centers that CSPNJ operates in the southern NJ counties, 43.57% of members identify as living in homelessness. Individuals who are interested in receiving assistance through the Housing Support Program must be enrolled in Medicaid, have a social risk factor such as current homelessness or recent incarceration, and a clinical risk factor like a mental or chronic health condition.

This initiative is made by possible by the collaboration of the New Jersey Department of Community Affairs (DCA) and Department of Human Services through the NJ FamilyCare Housing Supports Provider Readiness Program.
